Oak Gate Community

Since 1997, the Oak Gate Community, located on the third floor of Gleason Hall, provides dementia and early onset Alzheimer residents with quality care focusing on an individual’s unique needs. A creative, activity based, resident-centered program has been developed for persons in early stages of dementia who might need limited cuing, reassurance, and assistance with activities of daily living. Residents are assisted in maintaining the highest level of independence in the least restrictive environment 24 hours a day.

The program includes creative therapeutic and diversional activities seven days per week. Outings and group activities within the community provide cognitive, physical, recreational, spiritual and sensory stimulation. Family members of residents are always welcome and encouraged to participate in activities.
The community is designed for the following purposes:

  • encourage maximal participation in the activities of daily life;
  • provide activities that are specifically designed for cognitively impaired persons;
  • foster a sense of community;
  • provide round-the-clock presence and supervision to ensure resident safety.

Scheduled activities include: artwork, crafts, table games (cards, dominoes, bingo, etc.), baking, music, storytelling, scripture reflection, walks on Nazareth’s grounds, rides on Nazareth’s bus. Activities are scheduled at least four times a day to keep residents stimulated and engaged. Family involvement is encouraged.
