Assisted Living - Gleason Hall

Nazareth Living Center offers assisted living services to those who desire and request supportive living. Assisted living offers a unique mix of social, physical, and spiritual services in a secure environment.

Rates Effective 10/1/08:

Studio: $95.55-$100.75/day

One person - $133.85/day

The following services are included in the monthly rate:

• Medication oversight
• Three hot meals daily
• Special diets accommodated
• Tray service (when approved for medical reasons)
• Building and ground maintenance
• Housekeeping and linen service
• Transportation for planned facility activities
• Social, recreational and cultural programs
• Pastoral care services
• Daily Catholic Mass
• 24-hour emergency response system
• Utilities (except telephone)
• Cable TV
• Use of common and activities areas

Additional services and conveniences are available for a charge, including:

• Physical Therapy, Occupational Therapy, Speech Therapy
• Beauty and barber services
• Telephone
• Guest room (if available)
• Guest meals
• Personal laundry service

An additional $5.25-$10.50 fee per day will be assessed for residents requiring greater assistance for the following activities of daily living: toileting, bathing, transferring, dressing, and cuing for increased memory loss.

Assessment is required for all potential residents to determine level of assistance.
