The Right Place for Mom
"The staff and caregivers at Nazareth have a loving, caring, spirit. They know the residents and assist to keep them active, engaged in company,
and feeling safe. My mom has improved in spirit and physically living in this community. She loves the food, friends, and daily Mass. She
is content, at peace, and safe. She told me I found a good place for her."

Since 1997, the Oak Gate Community, located on the third floor of Gleason Hall, is a Special Care Unit which provides dementia and early stage Alzheimer's care for our residents focusing on an individual’s unique needs.
A Quality of Life Solution
A creative, activity based, resident-centered program has been developed for persons in early stages of dementia who might need limited cuing, reassurance, and assistance with activities of daily living. Residents are assisted in maintaining the highest level of independence in the least restrictive environment 24 hours a day.
The program is designed to encourage maximal participation in the activities of daily living and to foster a sense of community in an intimate faith based setting. Staff also provides round-the-clock presence and supervision to ensure resident safety.

Our aviary is enjoyed tremendously by our residents. It brings entertainment and a touch of home to many as the birds sing. As we have different species of birds, many of our residents have a favorite bird!
The Memory Care unit is managed by Janet Vise, (314) 487-3950 Ext. 4686, MSW, CDP and Certified Dementia Practitioner.
The campus has two aviaries, one of which is located in Gleason Hall. The other aviary is in McGovern Commons.
